Stephen Williams, DPT

Dr. Williams, Physical Therapist, has over 30 years of experience in physical therapy and holds a clinical Doctorate in Physical Therapy. He received his Doctorate in Physical Therapy from the prestigious Widener University, Chester-PA which was one of the first 10 in the country to offer a clinical Doctor of Physical Therapy degree. According to the American Physical Therapy Association, the Doctor of Physical Therapy is the preferred degree in physical therapy. Dr. Williams enjoys spending time with his wife Adetoyin and their four children, Emmanuel, Michael, Mark, and Elizabeth. He also enjoys watching and playing basketball. He is a dedicated fan of the Detroit Pistons.

Deborah Hayes

Intake Coordinator/Insurance Specialist

Deborah has over 10 years of experience in management of Outpatient Physical Therapy practices. She joined the Urbandale Physical Therapy team in 2010 as an intake coordinator/billing and insurance specialist. Deborah has her Associate of Applied Sciences in Health Information Technology. She also has certifications in both insurance billing and coding. Deborah can assist patients with scheduling, insurance verification, billing, community resources, and any other questions/concerns regarding your care. Deborah is married to her wonderful husband Jeff and is the proud mother of two boys (Trey who is 14 and Chad who is 6) and two stepdaughters (Kali 24, and Rachel 23). Deborah also has two dogs – Ben and Riley. Her hobbies include listening to music and anything outdoors. Deborah has a friendly disposition and is here to assist our patients in any way possible.

Brad Moore

Licensed Physical Therapy Assistant

Brad joined the Urbandale Physical Therapy team in 2013, after graduating magna cum laude from Baker College as a Licensed Physical Therapist Assistant. He loves working one-on-one with patients and has a fresh approach to providing quality care and working closely with the Physical Therapist. Brad is a fitness enthusiast and an avid outdoorsman who likes to spend his free time hunting, fishing, or at the gym. Brad and his fiancé Sara take multiple camping trips throughout the summer, even if it is for one evening. His favorite saying is “I would rather be lost in the woods than found in the city”. If Brad is not here at Urbandale Physical Therapy, he is out bonding with nature. Brad shares the same passion for his patients as he does for the outdoors.

Jason Neitzel

Licensed Physical Therapy Assistant

Jason joined the Urbandale Physical Therapy team in 2014, he has been a licensed PTA since 2012 after graduating from Kellogg Community College. Jason is constantly keeping up on the latest and greatest in the PT world and really enjoys working with all patients especially those with neurological issues. Jason is married to his wonderful wife Melissa and is a proud father of a 6 year old son, Noah. Jason likes to spend his free time with his wife and son and also enjoys music and movies. Jason believes that his hard work, along with the patient’s hard work, is the key to success.
